Objectives:
Understand the importance of addressing social drivers/determinants of health (SDOH) to improve comprehensive equitable health care and learn tactics to improve staff awareness with SDOH work.
Define health and explain different conceptualizations of population health and health equity
Describe strategies for how the six components of population health support high quality patient centered care and achievement of the Quintuple Aim

Speaker:
Angela Herman-Nestor, MPA, CPHQ, PCMH-CCE, joined the Missouri Primary Care Association in August 2004.  She serves as the Director of Health Care Transformation and Quality Initiatives for MPCA and holds advanced certifications as a Certified Professional in Health Care Quality and a NCQA Patient Centered Medical Home Certified Content Expert. Prior to joining MPCA, Mrs. Herman-Nestor worked in student health at a university, local public health agency, and state public health agency.
